Marc Jaynard V. Calivoso and Rolly Miguel Tabilas each filed an Affidavit of Loss in Tuguegarao City, Cagayan, Philippines. Calivoso graduated from high school in 2015 and lost his original diploma that was kept in his home in December 2020. Tabilas graduated from high school in 2003 and likewise lost his original diploma kept in his home in December 2020. Both individuals executed the affidavits to attest to losing their original high school diplomas and for any legal purpose the affidavits may serve.

AFFIDAVIT OF LOSS

I, MARC JAYNARD V. CALIVOSO, of legal age, Filipino, married,


presently residing at Batangan, Gonzaga, Cagayan, after being duly sworn,
hereby depose and state:
1. That on March 2015, I completed High School and graduated from
Ipil National Highschool, Gonzaga, Cagayan;

2. That as proof of the said completion I was issued a High School


Diploma by the said school;

3. That I placed the original copy of my High School Diploma inside a


brown envelop and keep it inside our house at my above mentioned
residence;

4. That sometime on December 4, 2020, I looked for the said original


copy of my High School Diploma but the same could not be found;;

5. That despite all efforts to find the said original copy of my High
School Diploma, I still cannot locate it and consider it lost;

6. That I am executing this affidavit to attest to the truth of the foregoing


facts and to use the same for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I hereunto affix my signature this 14th day


of December, 2020 at Tuguegarao City, Cagayan.

1. That on April 2003, I completed High School and graduated from Sta.
Teresita National Highschool, Sta. Teresita, Cagayan;

Diploma by the said school;

3. That I placed the original copy of my High School Diploma inside a


brown envelop and keep it inside our house at my above mentioned
residence;

4. That sometime on December 18, 2020, I looked for the said original
copy of my High School Diploma but the same could not be found;;

5. That despite all efforts to find the said original copy of my High
School Diploma, I still cannot locate it and consider it lost;

6. That I am executing this affidavit to attest to the truth of the foregoing


facts and to use the same for whatever legal purpose it may serve.
