WE, ABELARDO B. FAVILA and EMELVA M. FAVILA, of legal ages, husband


and wife, and residents of No. 18 Palanas C, Bgy. Vasra, Quezon City after having
been duly sworn to In accordance with law, depose and say:

1. That we are qualified applicants for the relocation of the National Housing
Authority (NHA) under the name Abelardo B. Favila;

2. That due to health and age, I Abelardo B. Favila being qualified applicant
hereby waive, transfer and relinquish all my rights and interest over the said
relocation in favor of our daughter EMEBELLE F. FRANCO on our behalf.
