AFFIDAVIT

I, KING’S BARGAIN CORPORATION a business duly organized and


existing by virtue of the laws of the Republic of the Philippines represented
by ANN WENDY QUIMBO, of legal age, Filipino, married and a resident of
Ormoc City, Leyte, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am the treasurer of the abovementioned corporation with


business address at Corner P. Burgos and Rizal St(s), Ormoc City,
Leyte, Philippines;

2. That the following names are not connected in the aforementioned


corporation and the same are bona fide member of Social Security
System (SSS) namely:
MADEL BONCALES (SSS No.:06-22018014
JESABEL INDOLOS (SSS No.:0631214003

3. That MADEL BONCALES is no longer connected staring April 30,


2018, and JESABEL INDOLOS starting June 22, 2018;

4. I am executing this Affidavit to attest the veracity of the foregoing


facts and for whatever legal purpose it may serve best.
