JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, EDITHA S. TAGOLINO and ERLINDA M. PEPITO both of legal age,


Filipinos, and both residents at Ormoc City, Leyte, Philippines, after having been
duly sworn to in accordance with law, do hereby depose and say that:

1. We personally knew ELEUTERIO R. CERILLA who was born on May 3,


1943 at Pilar Camotes, Cebu and our longtime friends and acquaintance
in Ormoc City;

2. The name ELEUTERIO R. CERELLO was erroneously entered thereto


appearing to his Certificate of Marriage at the office of the City Civil
Registrar of Ormoc City and the name ELEUTERIO R. CERILLA
appearing to his other pertinent documents or specifically to his SSS ID
bearing ID number 06-0489580-2;

3. We are not in way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have pecuniary
interest in this matter whatsoever;

4. That we hereby declares under oath that the name ELEUTERIO R.


CERELLO and the name ELEUTERIO R. CERILLA, refer to one and the
same person;

5. We are executing this affidavit to attest the veracity of the foregoing facts
and for whatever other legal purpose this serves best.
