AFFIDAVIT OF NON-OPERATION

I, _______________________ of legal age, married, Filipino, and a resident of


Astorga Proper, Sta. Cruz, Davao del Sur, after having been sworn to in accordance
with law hereby depose and say:

1. That I am the same and identical person who cause the execution of this
instrument;

2. That I am the Sole Proprietor and owner of ________________, a business duly


registered under existing laws of the Republic of the Philippines with business
address at Astorga Proper, Sta. Cruz, Davao del Sur, DO HEREBY CERTIFY:

3. That since February 2015, the aforesaid business has not yet started its operation
nor commenced the transaction of its business until the present date;

4. That I am executing this affidavit to formally inform the Bureau of Internal


Revenue (BIR), Digos District Office, Davao del Sur and to attest to the truth of
the foregoing facts and for whatever legal purpose this may serve.
