JUDICIAL AFFIDAVIT OF THE WITNESS
(For the Plaintiff)
I, RONILO U. BORJA, Filipino, of legal age, residing at #123 Ramos St., Brgy. San
Vicente, Tarlac City; after having been sworn in accordance with law depose and state that:
PRELIMINARY STATEMENT
1. Atty. Jonathan Pacifico personally conducted and supervised my direct examination at his
office at 01 Hilario St, Brgy.Ligtasan, Tarlac City, 2300 Tarlac; and
2. I am answering the questions asked of me based on my personal knowledge, fully
conscious that I am doing so under oath, and that I may face criminal liability for false
testimony or perjury.
OFFER OF TESTIMONY
The testimony of witness RONILO BORJA is being offered to prove the allegations
contained in the Information for ______________ against Valentino Baldovino.
Question No. 1 : Please state your name and other personal circumstances.
Answer : I am Ronilo Borja, of legal age, married, and a resident of
#123 Ramos St., Brgy. San Vicente, Tarlac City. I am
currently working as a City Social Welfare and
Development
Officer at the City Social Welfare Development Office at P.
Burgos St., Tarlac City.
Question No. 2 : What do you do as a City Social Welfare and Development
Officer?
Answer : One of my function as a City Social Welfare and
Development Officer is to facilitate the implementation of
welfare program for disabled, elderly, and victims of drug
addiction, the rehabilitation of prisoners and paroles, the
prevention of juvenile delinquency, and such other
activities which would eliminate or minimize the ill-effects
of poverty.
Question No. 3 : What is your function with regards to Children in Conflict
with Laws or CICL?
Answer : After arrest and after the initial investigation has been
conducted by the police officers, the CICL must be turned
over to my custody within eight (8) hours. I would then
explain in simple terms to the child and his/her
parents/guardians the consequences of the child’s act.
Question No. 3 : Do you know Valentino Santillan Baldovino?
Answer : Yes sir.
Question No. 4 : How do you know him?
Answer : I was the social worker to whom he was referred to after
the initial investigation. I likewise assessed him if he has
acted with discernment or not.
Question No. 5 : What is your findings?
Answer : After careful study of all the facts and circumstances of the
case, taking into consideration the educational level and
performance of the child and his appearance, attitude,
comportment and behavior before, during and after the
commission of the offense, I put in my Case Study Report
that Valentino Baldovino acted with discernment.
Question No. 6 : Is this the Case Study Report that you are pertaining to?
Answer : Yes sir.
Question No. 7 : Can you identify the signature appearing at the last page of
this document?
Answer : Yes sir, this is my signature.
