Greetings!

I, John Christian P. Ramos, of legal age, resident of 2257 Int 8 Callejon Felipa St. Sampaloc Manila,
am asking your good office for a Medical Assistance grant.

I am currently diagnosed with Chronic Kidney Diseases Stage IV secondary to Chronic


Glumerulonephritis and Diabetes Milletus Type II with other complications (please see attached
medical abstract coming from my Nephrologist). I’ve been suffering from this disease since 2009 and
because of the nature of the disease it progresses to its later stage. I have been prescribed with
different medications as treatment for my liver, heart and kidneys that have been affected my CKD,
and some of the medication are costly and I cannot afford to purchase everything with my current
status (please see attached doctor’s prescription).

Our family are still trying to make ends meet, with the new medications prescribed to me we can’t
afford anymore to purchase these medications hence we seek your assistance.

I am also authorizing my mother, Ma. Anna Marian P. Ramos to transact business in my behalf due
to me being confined with responsibilities and this disease.

I attached other pertinent documents for your reference needed for my request.

I am hoping that you could assist us and grant our request of medical assistance.
