Mababanaba National High School: Dr. Imelda P. Macaspac

This document is a request from Mary Ann D. Salgado, a teacher at Mababanaba National High School, to transfer to Sapang High School. She provides four reasons for the transfer request: 1) She is a resident of Pacpaco San Manuel and finds the commute difficult. 2) The transfer would save her money from weekly commuting costs. 3) While assigned far from home, she is willing to perform her duties effectively. 4) As the sole breadwinner for her family, the transfer would help financially support her parents who depend on her. The principal of Mababanaba National High School noted the request, which was then approved by the Schools Division Superintendent.

Madam:

I, MARY ANN D. SALGADO, a resident of Pacpaco San Manuel, Tarlac assigned

Teacher I at Mababanaba National High School, Mababanaba, San Jose, Tarlac, sincerely
requesting for transfer to Sapang High School due to the following reasons;

1. I am a bona fide resident of the municipality of San Manuel and it’s not easy for me
to commute.
2. For economic reason, I can save a lot from weekly fare going home and back.
3. Though I am assigned far from my hometown (San Manuel), I am willing to perform
my duties and responsibilities as classroom teacher effectively and efficiently but
opportunity knocks in which is the Division Memorandum No. 229, s. 2018 so I
fervently hope for my transfer.
4. Lastly, I am the breadwinner of my family where my parents are depending on me
since I am their only child.

I earnestly pray that this request merits your most favourable action. Thank you and God
bless.

Respectfully yours,

MARY ANN D. SALGADO

Secondary School Head – Head Teacher I
Mababanaba High School

The undersigned is a bonafide resident of Barangay Bulo, Victoria, Tarlac and is recently
employed in the Department of Education for three years on June 22, 2018. I am stationed at San
Juan de Valdez Integrated School and have been requesting to transfer for a teaching position at
Mababanaba High School.

I graduated at Tarlac Christian College with a degree of Bachelor of Secondary Education –

Major in English with units in Master of Arts in Education – Major in English. I am inspired to
pursue teaching, because of my strong desire to positively influence and educate youths.

I was deployed at Mababanaba High School in June 2015, though I was assigned at San Juan de
Valdez Integrated School. Currently, I am teaching English 10 at MHS. I am also the School
Paper Adviser of The Hillsiders (the official school publication of MHS in English). There are
loads of works to do, however, I am able to accomplish the required tasks on time and face the
challenges with good results. I know it develops and enhances my personality. It also leads to
professional growth and proves that dedication, patience, understanding, compassion, and
sensitivity are essential to this profession.

I am aware of the challenges I am about to face

7in this line of work, but I believe that perseverance together with my determination to teach
will make learning fun and enjoyable for my students.

Thank you for taking time to consider my application for transfer. I look forward to hear from
you, and I will do my best to help you and our youth.

Respectfully yours,

Maricar Q. Dela Cruz
