I, FAYE S. RAMOS, of legal age, married, Filipino, and a resident of


#96 T. Delos Santos St., Science City of Munoz, Nueva Ecija, after having
been duly sworn to in accordance with law, hereby depose and state, that:

1. I am the owner/proprietor of dry goods business selling RTW clothes


located at the public market of ____________________________.

2. That I am earning a net monthly income of Ten Thousand Pesos


(P10,000.00) in the operation of the said store.

3. I am executing this Affidavit in order to attest to all the foregoing


statements, to prove that my source of income is the earnings from my
