AFFIDAVIT OF SERVICE

I, ADONIS FEL U. FLORES, of legal age, a Filipino Citizen, and a


resident of University of Eastern Philippines, University Town, Catarman,
Northern Samar, Philippines, after having been duly sworn to in accordance
with law do hereby depose and state:

1. That I am the respondent on the Preliminary Investigation under the


Office of the Provincial Prosecutor, Laoang, Northern Samar with NPS
Docket No. VIII-11b-INV-20B-00023;

2. That I served a Motion for Reconsideration by registered mail


pursuant to Section 7, Rule 13 of the Rules of Court to Joy A. Flores,
the complainant;

3. That I am serving the Motion for Reconsideration through registered


mail because the complainant refused to receive it through personal
service.
