AFFIDAVIT

I, LINO P. PAHIMNAYAN, of legal age, Filipino citizen, married and


a resident of Brgy. Chitongco, Mondragon, Northern Samar, after having
been duly sworn to in accordance with law do hereby depose and say:

1. That I was not able to claim my Letter of Introduction (LOI) and


ATM on time and as such the same was delayed;

2. That consequently I claimed my aforesaid Letter of Introduction


and ATM;

3. That I am executing this affidavit to attest to the truth of all the


foregoing statements and to depose under oath the delay of my
claim regarding my LOI and ATM which could have been purely
due to inadvertence and for whatever legal purposes this may
serve.

IN TRUTH WHEREOF, I have hereunto set my hand this 19 th day of


December 2022 at Catarman, Northern Samar.
