This affidavit was made by Jaycee Resurreccion Perea to correct the entry of his sex/gender on his birth certificate from "FEMALE" to "MALE". He was born on January 24, 1988 in Palumlum, Alfonso, Cavite and his birth was registered with the local civil registrar. However, upon securing a copy of his birth certificate, he found that his sex was erroneously indicated as female when he is actually male. AFFIDAVIT

I, JAYCEE RESURRECCION PEREA, of legal age, Filipino, single with


present address at Brgy. Palumlum, Alfonso, Cavite, after having been sworn to in
accordance with law, hereby depose and state, that:

1. That I was born on January 24, 1988 at Palumlum, Alfonso, Cavite and a
Certificate of Live Birth was registered with the Local Civil Registrar of
Alfonso, Cavite;

2. That when I secured a Local Civil Registry copy of my Certificate of Live


Birth with Registry No. 88-119, I found out that the entry for my
Sex/Gender was erroneously indicated as “FEMALE” when in truth and
in fact I am a “MALE”;

3. I am executing this affidavit to attest to the truth of the foregoing


statement and for the purpose of solemnization of marriage and for
whatever legal purpose this may serve.

IN TRUTH WHEREOF, I have hereunto affixed my signature this ___th day of
