AFFIDAVIT OF CONSENT

I, SHECEL JOYCE TAGUD , Filipino, single , of legal age, and presently


residing at Tinago , Inopacan , Leyte respectively, having been duly sworn in
accordance with law, hereby depose and state that:

1. I am the daughter of Celso Tagud of Inopacan , Leyte who died on

May 29,2020 at Dammam , Saudi Arabia ;

2. Pursuant to existing laws, policies, rules and regulations, including


DOH DM 2020-0067 dated February 3, 2020, DILG MC 2020-063 dated March 27,
2020, DOH DILG JMC No. 1 Series of 2020 dated April 16, 2020, DOH-BOQ MC No.
2020- 023 dated July 1, 2020, and to ensure infection prevention and control, and
uphold public health and safety, I hereby signify my written consent, freely,
willingly and voluntarily to have the remains of Mr. Celso Tagud be cremated in the
designated crematorium establishment/s of ___________________________ before
they turn-over to us the remains of our cadaver;

3. Further, we are freely giving our full consent to


_________________________ who is the designated staff of
__________________________________ to disclose the appropriate personal
information of the deceased and its preferred and legitimate claimants to the
appropriate government agencies in facilitating: a) the cremation and transport of
the remains of our cadaver; b) turn-over of the remains to the claimants; c) identify
the potential beneficiaries who can avail of the death compensation benefits offered
by various government agencies; and d) such other matters;

4. I fully understand the contents of this Affidavit of Consent and I have


full knowledge of its consequences. Also, the content and meaning of this document
has been fully explained to me in a dialect and/or language known to me; and

5. I am executing this affidavit to attest to the truth of all the foregoing


statements and for whatever legal purposes it may serve.
