AFFIDAVIT OF EXPLANATION

I, PCpl Melfe Cabañesas Agcopra, designated as Asst MCAD/FJGS PNCO of


Initao Municipal Police Station, Initao Misamis Oriental, after having been sworn to in
accordance with the law, do hereby depose and state:

1. That, on May 2, 2020, we conducted a whole day scheduled MCAD


activities
specifically Mangrove Planting, Coastal Clean-up, Packing and distribution of Goods
under the Adopt a Family Program and other MCAD related activities in Barangay San
Pedro, Gimangpang and Pontacon respectively;

2. That, at around 6:30 AM, the undersigned together with other personnel of
Initao MPS, Municipal Advisory Council and BPATs started the Mangrove Planting
followed by a coastal clean-up held at P-1, San Pedro, Initao Misamis Oriental;

3. That, thereafter, the undersigned together with Pat John Carlo A Galarosa,
Asst. MCAD PNCO, Pat Crislyn Amad Naquin, Beat Patrol, NUP Laarni Joy J
Conadera
and NUP Merlyn H Sancho proceeded to our next activity in relation to Adopt A Family
Program which is the packing of relief goods held at Cabañesas residence, P-1, San
Pedro, Initao Misamis Oriental;

4. That, after the packing of relief goods, the abovenamed personnel together
with PSSg Jarry S Suarez, Supply PNCO / Driver headed to Barangay Gimangpang
specifically at Sitio-Sta Cruz and P-Matinabangon for the distribution of relief goods to
less fortunate families and children under the Adopt a Family Program. The activity
ended on or about 11:45 o’clock in the morning.

5. That, the group headed back to Initao Municipal Police Station for a lunch
break;

6. That, on or about 2:00 o’clock in the afternoon, the undersigned together


with
same personnel headed to Barangay Pontacon to conduct distribution of relief goods
to less fortunate families in relation to Adopt a Family Program and gift giving
program, an initiated best practice of Initao MPS to the children of said barangay .
The activity ended on or about 5:20 o’clock in the afternoon;

7. That, due to the abovementioned MCAD scheduled activities, I was not able
to present myself in Quarantine Control Point (QCP) when the Regional
Director visited thereat.

8. That, the undersigned have attached herewith the documentation of said


activities conducted;

9. That, the undersigned is hereby executing this Affidavit to attest the truth
and
veracity of the foregoing statements and prays that this explanation be acceptable and
given merit;

10. Other relief, just and equitable is also prayed for.
