AFFIDAVIT OF LOSS

I, EMILY CABANTAC FRIBERG, of legal age, Filipino, married and a resident


of Carmen, Cagayan de Oro City, after having been sworn to in accordance with law do
hereby depose and say that:

1. I am bona fide member of FIRST COMMUNITY COOPERATIVE (FICCO) –


Tiano Branch, Cagayan de Oro City, with ID No. 054364, of which I am maintaining,
among others, a Savings Account and I was issued of a Passbook for the said account;

2. I lost the said passbook for my Savings Account the date, time and
circumstances of which I could no longer remember. And despite due and diligent
effort made and undertaken to retrieve the passbook, such effort proved futile and
hence I truly believe that the passbook is beyond recovery;

3. The said Savings Account Passbook has not been confiscated by any officer of
the law or the issuing office or agency due to the violation of any law, statute, order,
rule or regulation. And in the event that the said Savings Account Passbook should
hereafter be found or located, I undertake to forward or surrender the same to the
proper authority for appropriate disposal;

4. That I am executing this Affidavit to attest to the truth and veracity of the
foregoing statements and for any legal purpose this Affidavit may serve.

IN WITNESS WHEREOF, I have set my hand this December 14, 2020, in the
City of Cagayan de Oro, Philippines.
