BARANGAY CERTIFICATION OF ACCEPTANCE

This is to certify that ISIDRO GATON JR., DRIZZLE ALOG, ZIAN JULS
SANCHEZ and SEELENA GATON has been given permission to travel back to Barangay
Buenavista, Irosin, Sorsogon.
They will board a Black Nissan NV350 with Plate Number F2T 345 and Driven by
Adrian Adriano from UpperBicutan. They are ordered to undergo triange in the Municipal
Quarantine facility at the Gallanosa National High School for confirmatory check-up and
subjected to the guidelines and protocols under MGCQ. They will be cleared to return to
Barangay Buenavista once the Municipal Health Office orders him to do so.

Further, Home quarantine shall only be allowed subject to all of the following provisions in
accordance with the Provincial Executive Order No. 78-2020 and Provincial Order No. 11-2021:

 Has a negative RT-PCR result within 24 hours between the date of release of result and
arrival in Irosin, and is asymptomatic upon arrival.
 The MHO and/or barangay Council has inspected and deemed the place fit as a home
quarantine location and compliant to DOH standards.
 The persons undergoing home quarantine shall be placed in a separate room with its
own assigned toilet and bath from the rest of the household and there are no vulnerable
individuals in the household;
 The persons undergoing home quarantine shall be prohibited to leave their rooms/homes
where they are quarantined until they have been certified by the MHO to have finished
the required the quarantine duration; and
 The persons undergoing home quarantine shall be monitored by the respected Barangay
Health Emergency Response Team (BHERT),personnel assigned for the task.

This certification is issued upon request of the interested party for whatever legal purpose it
may serve.

Issued this 23rd day of March 2021 in Barangay Buenavista, Irosin, Sorsogon.
