This QUARANTINE CERTIFICATION/CLEARANCE are GRANTED To Mr./Ms. PRINCESS R.


CORPUZ, from Barangay Longos Central, San Fabian, Pangasinan, due to enhance quarantine
implemented, the jurisdiction of travelling to other places specifically in affected places of COVID-19
are limited, by follow the protocol given by the D.I.L.G.,

I, CORAZON S. ERESTINGCOL, Brgy. Captain of the said Barangay, GRANT a


CLEARANCE/CERTIFICATION to the said Constituent/s that he/she finished the “14 day quarantine
period” , don’t have any symptoms of COVID-19 in time of quarantine, with safety precaution,
undergo sanitation and proper procedure of the quarantine.

This certification was issued upon request of the above name person for any/whatsoever legal
purposes it may serve best.

Given this 6th day of JUNE, 2022 in Barangay Longos Central, San Fabian, Pangasinan.
