Torreflores executed an affidavit of loss to declare that his Philippine Army Finance Center Producers Intgegrated Cooperative (PAFCPIC) passbook was misplaced in February 2020 during an assignment in Sagay City, Negros Occidental. AFFIDAVIT OF LOSS

I, JHEYD H. TORREFLORES, of legal age, single, Filipino citizen and a


resident of Brgy. Binaliuan Mayor, Tigbauan, Iloilo, Philippines, after having
been duly sworn to in accordance with law do hereby depose and say:

1. That I am a bonafide holder of one (1) Philippine Army Finance Center


Producers Intgegrated Cooperative (PAFCPIC) Passbook issued by PAFCPIC-
Jamindan, Capiz Branch;

2. That the afore-mentioned PAFCPIC Passbook was misplaced during my


assignment at Sagay City, Negros Occidental on February 2020;

3. That I exerted diligent efforts to look for the said PAFCPIC Passbook but to
no avail hence, I consider the same already lost and beyond recovery;

4. That I am executing this affidavit to attest to the truth of the foregoing and
to declare that the said PAFCPIC Passbook is already lost and beyond
recovery and to request PAFCPIC-Jamindan, Capiz Branch, to issue a new
Passbook in my favor;

5. That further, I am executing this affidavit for whatever legal purpose it may
serve.
