AFFIDAVIT OF LOSS
I,CAMILO ANGUID, of legal age, Filipino, with residence and postal address
at Maruing, Lapuyan, Zamboanga del Sur, after having sworn to in accordance to
law, we hereby depose and sayeth - THAT:
1. I am an owner of a Chainsaw with STIHL Brand bearing Serial No.
410195959 made in Germany;
2. It is not brand-new as I have purchase it from another person but still
in good condition;
3. I applied for a permit at the Office of the Department of Environment
and Natural Resources to operate the said chainsaw and one of the
requirements it the receipt of such purchase;
4. Unfortunately, as I inquire from the person whom I bought the
chainsaw for the receipt, he told me that it was a long time ago since
he purchased it and have misplaced or lost the receipt thereof;
5. Despite arduous and diligent efforts of locating it, it was found futile;
6. I am executing this affidavit to attest to the truthfulness of the
foregoing facts and in support of my application for the issuance of
permit to operate said chainsaw and for whatever legal purposes it
may serve;

174165874 made in Germany;
2. I applied for a permit at the Office of the Department of Environment and
Natural Resources to operate the said chainsaw and one of the
requirements is the receipt of such purchase;
3. Unfortunately, as I look upon for a place where I usually put the said
receipt of my purchase, it was not there anymore, I believe that it might
have been thrown away, believing that it was a scratch, despite arduous
and diligent efforts of locating it, it was found futile;
4. I am executing this affidavit to attest to the truthfulness of the foregoing
facts and in support of my application for the issuance of permit to
operate said chainsaw and for whatever legal purposes it may serve;
