JOINT AFFIDAVIT OF CONSENT

married and widow respectively, all Filipinos, of legal ages, and with postal address at
____________________, Philippines, after having been duly sworn to in accordance with law, do
hereby depose and say:
1. That we are the actual co-owners and possessors of an unregistered parcel of land
designated as _____________, situated in the Barangay of __________, Municipality
of ___________, Province of Bohol, Island of Bohol per Sketch Plan prepared by
Geodetic Engineer, ____________________, and containing a total area of SIX
THOUSAND ONE HUNDRED SIXTY-SIX (6, 166) square meters more or less;
2. That our nephew, _________________, wants to build a residential house on a portion
of ________________ which is the share of the inheritance of his mother,
__________________;
3. That we give our full consent to ________________, likewise Filipino, of legal age,
married, and with postal address at ____________, Bohol, for the construction of a
residential house on a portion of _______________ including the installation of
electrical service and/or post by the BOHECO I and water service connection by the
Local Government Unit of Panglao. Thus, he is applying for a Building Permit before
the Office of the Building Official of Panglao, Bohol;
4. That we are executing this affidavit to attest to the truth of the foregoing facts in order
to support the application for Building Permit for the construction of residential house in
compliance with the requirements set forth by the Local Government Unit of Panglao,
Bohol and BOHECO I, and for whatever legal intents and purposes this may serve.
