having been duly sworn to in accordance with the law, do hereby depose and
state:

1. That I am the Buyer of a mixed-used, Commercial-Residential type


condominium unit located at the (DETAILS OF DEED OF ABSOLUTE
SALE), as evidenced by a duly executed Deed of Absolute Sale dated
October 10, 2022 entered in the Notarial Registry of Atty. (name of
notary public), CPA, as Doc No. 123; Page No 12; Book No. XXX; Series
of 2022 (the, “Deed”);

2. That in the said Deed, my civil status does not appear.

specific purpose of transferring said property to my name with the Office
of the Registry of Deeds, Taguig City.
