The terms of our agreement are as follows:

At the time of enrollment the undersigned parent(s) or guardian(s) understand that care will be
billed at the rate of $__________ bi-weekly tuition fee for their child. Based on care being
provided on the days provided on the teacher schedule form for each term.

Fees are to be paid bi-weekly, by the means of electric money transfer (E.M.T.) See schedule on
the daycare website: daycarewebaddress.com or ask for a printed copy for payment dates. All
fees are due by 5:00pm on the scheduled pay day.

Payment is your tuition for the program and does not vary due to parental vacation, or illness of
the child. If the daycare is open and your child does not attend for whatever reason, or if the
daycare is closed for any reason, payment is still expected in full. In other words you pay the
same tuition fee amount every two (2) weeks.

A non-refundable enrollment fee of __________ is required. The enrollment fee is required at

the time this agreement is signed by both parties (parent(s)/guardian(s), and caregiver) and it
could be applied to the child’s last 2 weeks of care ALREADY COMMENCED and/or to cover
other cost depending on termination (see termination section of the Hand Book for more details).

Acknowledgement of Policies
I, ______________________________ and __________________________ have read and
understood all Policies and Guidelines of Daycare Name run by Providers Full Name.

____________We (I) agree to abide by all polices that are listed on the Daycare website;
daycarewebaddress.com, Parents Hand Book and Health & Safety Hand Book. We (I) have read
and understood these policies before signing this contract.

____________We (I) understand that we will be notified in writing of any changes in any of
these policies.

____________We (I) understand that any complaints, concerns, or grievances against the
daycare, the provider, any of the provider’s family or any other clients of Daycare Name, must
be made in writing and will be followed up in a timely manner.

____________We (I) agree to paying 100% of replacement costs for any damages that occur due
to intentional damages of Daycare Name (Providers full name) property through destructive
behaviour, roughness, or due to a breach of one or more of my policies listed in the hand book,
and on the Daycare website. Reimbursement will be paid within a reasonable time frame set by
the provider.

____________We (I) understand that daycare is a business. We (I) will not bring in any personal
issues or beliefs into the daycare. We (I) will only bring in positive information that will
contribute to our (my) child’s well-being and care.

____________We (I) are aware that if any payments of fees or reimbursement costs are not
made as stated in this contract, or on the Daycare website, Parents Hand Book and Health &
Safety Hand Book, than daycare name has the authority to take us (me) to Small Claims Court or
to have a Collections Agency collect those fees on behalf of Providers full name. We (I)
understand that we are responsible for fees in evolved in either going to Small Claims Court or
Collection Agency.

____________We (I) understand that any breach of policies may be grounds to terminate
childcare. A two week notice will be given in such circumstances unless the infractions are
severe enough to warrant termination without notice.

____________We (I) understand that any money paid will NOT be refunded for any reason
should there be a termination of services by either party.

____________We (I) understand that the daycare can increase the tuition fee when the contract
is renewed (December for regular care, August for Teacher care).

____________ We (I) understand that payment is done by flat weekly tuition fee and does not
vary due to any reason. In other words we (I) pay the same amount every two (2) weeks.

____________We (I) understand it’s our (my) responsibility to read all on the daycare website,
and in the copy of Parents Hand Book and Health & Safety Hand Book given to us (me).

____________We (I) understand that it is our (my) responsibility to ensure that the tuition fee is
paid in full and on time.

____________We (I) understand that outside of the daycare area (inside/outside of the building)
is a private home/property. We (I) will respect the private areas.

____________We (I) understand that neither the daycare nor the provider holds a daycare
license. THIS CHILD CARE PROGRAM IS NOT LICENSED BY THE GOVERNMENT OF
ONTARIO

____________We (I) understand that the daycare requires an updated copy of our (my) child’s
immunization record. We (I) understand that our (my) child can NOT start care until the copy of
the immunization record has been provided.

____________We (I) understand that The Ministry of Education has the right to enter the Home
Daycare without notice. The Ministry has the right to do, but not limited to, the following:
- Record audio and video of all the children and other persons in the daycare and home.
- Take still photos of all the children and other persons in the daycare and home.
- Look at and take copies of all the daycare forms, including contracts, and other forms with your
personal Information on them.
- Contact you for questioning, even after care has ended.

____________ We (I) understand that All caregivers are required by law to report any suspected
cases of child abuse or neglect. We (I) will not sue, press charges, threaten or slander Daycare
name, (Providers full name or any of the provider’s family members) if they follow the law by
reporting any cases of suspected child abuse or neglect.

____________ We (I) understand that from time to time there will be an alternate provider and
or assistant (18yrs+) watching our (my) child well the provider attends meetings, appointments
etc.

____________We (I) understand that the daycare holds the right to change any policy at any
time in writing with the following notices:

- Health: All Health policies can be updated/added without notice at any time in order to
protect the health and safety of your child, the other children in my care, my family and myself.

You will receive a written notice at either drop off or pick up stating that updates have started
immediately.

- Safety: All Safety Policies can be updated/added without notice at any time in order to
protect the safety of your child, the other children in my care, my family and myself. You will
receive a written notice at either drop off or pick up stating that the policy updates have started
immediately.

- General: All General Polices can be updated/added with a two week notice. You will
receive a letter at either drop off or pick up stating the changes and when the policy comes into
effect.

____________We (I) understand that we (I) are responsible to update the provider if any of our
emergency contact information changes or if your contact information changes.

___________We (I) understand that ALL contents of Daycare Name contract, website, and
handbooks are final, binding and non-negotiable. Any attempt by clients to argue, bully or
renegotiate contract stipulations, policies, or procedures can and will result in termination of this
contract. The decision to make an exception by the provider in one instance, does not negate
future requirements to abide by contract.

____________We (I) understand that all information regarding our (my) child’s care will only
be given to us (me); the legal guardian(s). This applies even when a third party is paying the
daycare fees.

____________We (I) understand that Daycare Name is located in the provider’s home. This
means form time to time family members and friends might be staying in the home or stopping
by for a visit. This includes being in the home during daycare hours.

____________We (I) have informed the provider of any known health/medical issues that our
(my) child has or develops.

____________We (I) understand that all fee and contract information is confidential and is not to
be discussed with other clients (past, current, potential), or anyone other than Providers full name

____________ We (I) understand that in an emergency situation the provider can NOT
guarantee who will be watching your child. Please note that there will always be an adult present.

____________ We (I) understand that ONLY one parent/legal guardian needs to sign the
contract, and any forms at time of signing and throughout our (my) child’s care at Daycare
Name. We (I) understand that both parents/legal guardians are responsible to abided by all the
policies and financial agreement, unless otherwise stated in writing at the time the contract is
signed.

signing.

____________We (I) understand that this contract is not valid until signed by the provider, the
enrollment fee has been received in full and the daycare has received a copy of our (my) child’s
immunization record and our (my) child’s birth certificate.

____________ We (I) understand that this contract replaces any and all other contracts that come
before it.

___________ We (I) understand that if a new handbook(s) is given to us at the time of signing
this contract, the new handbook(s) (with the date it come into effect on it) will replace any and
all handbooks that come before it.
