AFFIDAVIT OF LOSS

I, ALMA QUIAMBAO TONZO, of legal age, Filipino, resident of and


with postal address Shangrila Homes, San Jose, Tarlac City, after having
been duly sworn to in accordance with law do hereby depose and say:

1. That I am the holder of a Passbook with Account No. 9024064188


issued by RCBC Savings Bank;

2. That sometime on July this year, the above-said passbook was


lost/missing;

3. That despite diligent efforts exerted to locate, the same could no


longer be found and is now deemed lost and beyond recovery;

4. That I am executing this affidavit to attest to the truth of the foregoing


statements and facts to inform the concerned agencies.

AFFIANT SAYETH NAUGHT.
