I, HON. ARNOLDO O. CASTILLO, legal age, Filipino and a resident of


Matas Compound, Almendras District, Padada Davao del Sur, Philippines,
respectively after having been sworn to in accordance with law, hereby
depose and state;

That I am the same and identical person who caused the execution of
this affidavit.

That I am the Punong Barangay of Almendras District, Padada, Davao


del Sur, Philippines;

That I know for a fact that a certain MS. JASAREL J. SARDIDO with
Community Tax No. 27451515 issued on June 08, 2010 at Almendras District,
Padada Davao del Sur,is a bonafide resident of Purok 4, Almendras District,
Padada, Davao del Sur.

That I am executing this affidavit freely and voluntary in order to


appraise proper the Schools Division Superintendent of Davao del Sur of the
veracity and truthfulness of the foregoing averments;

sworn to in accordance with the law, do hereby depose and say:

That; I am the same identical person who caused the execution of this

That; I together with my family have been actually and continuously


residing at Purok 4, Almendras District, Padada, Davao del Sur since birth
until at present:
