AFFIDAVIT OF LOSS

I, JOVITA B. HUGO, a Filipino citizen, of legal age and with residence


address at Lumbang, Isabela City, Zamboanga Peninsula, Philippines, after being
duly sworn to in accordance with the law, hereby depose and state:

1. That I am a member of the Pantawid Program of the Department of Social


Welfare and Development and was issued an ID;

2. However, due to inadvertence, said ID got lost sometime I cannot remember;

3. That despite diligent search and efforts to locate my ID, I could not find
the same such that I now believe that it is now lost beyond recovery;

4. That I execute this affidavit in order to attest to the truthfulness of


foregoing narration of facts and for whatever legal purposes this may
serve better.

IN WITNESS WHEREOF, I have hereunto set my hand this day 4 th day of


February, 2021 at Isabela City, Zamboanga Peninsula, Philippines.
