I, Mohdf Rafi Shah S/oMohd Yousf shah R/oKhanoo babagund tehsil


Handwara Disii Kupwara here by solemnly affirm on oth as under.

That the applicant is registered as beneficiary under bullding and other


construction works welfare bord (Registering officer,district,Srinagar)
under registration No134 Dated 08.06.2011

2. that the applicants marriage has taken Place on-17.10.2011at Miqam I


shahwali Drugmullah Kupwara With shreefa BanoD/o Gh Ahmad PirR/o
Muqam-i- Shahwali.

3. That the marriage of my son namely Mohd Rafi shah has taken place
on 17.10.2011 with shareefa bamo D/o gh ahmad Pir R/o Muqami-I
Shahwali at Muqam.i Shahwali.

4. That the applicant is presently works as Carpenter since 7 years and I


am Physically fit for performing my work.

5. That I solicit this affidavit for marriage assistance before the building
and other construction works welfare board.

6. That if anytime it is provide that I am not building and other


construction worker/ the application for marriage assistance is ultimately
found false I am personally responsible for legal implication thereof and
will refund the amount received as marriage assistance from the Board.

Handwara do hereby solemnly declare on oath as under:

1. That my Husband has passed away from the last three years and I am
the only legal heir of the said deceased.

2. That my husband was account holder of Jammu & Kashmir Bank Ltd.
Branch Langate bearing account No. 3411/SB in which my husband has
deposited an amount of Rs. 20079/= (Rupees Twenty thousand and
seventy nine only).

3. That I have to denote the said amount of Rs. 20079/= (Rupees Twenty
thousand and seventy nine only) to Markazi Jamia Masjid Sharief
Langate, the same may be released in favour of the Masjid Committee.
