AFFIDAVIT

I,_____________ of legal age,married, PNP member and presently assigned at Sta


Praxedes Police Station \ Cagayan after having been duly sworn to an oath in accordance with
law do hereby depose and say;
That on July 20, 2022 at around 10:00 AM, I was informed \ was involved into a
commotion against two (2) victims namely___________ and ____________.
That earlier before the incident, I noticed that was already off duty. Unknowingly that on
his way home, he had self-inflicted accident in \ that resulted in the damaged of his driven
vehicle.
That due to the damaged incurred of his vehicle, he was not able to get home, instead,
he went to see a mechanic for the repair of his vehicle and he decided to stay and return to this
office.
hence, this office cited that was not officially on duty during the day of that incident that
charged him a grave slander by deed.
Furthermore, due to the complaints of victims against him, this office keeps him under
custody while preparing the appropriate case.
