AFFIDAVIT OF DENIAL

I, , of legal age, married, Filipino citizen and a resident of, Philippines after
having been duly sworn to in accordance with law, do hereby voluntarily depose and
state:

1. That I am applying to study abroad;

2. That one of the requirements is to secure clearances from the proper


authorities;

3. That when I went to secure a clearance from the National Bureau of


Investigation (NBI), I was surprised to learn that there are cases for violations
of the Comprehensive Dangerous Drugs Act (RA 9165) particularly Sections 11
and 12 with Criminal Case Nos. 161113 and 23 before the RTC Branch with the
name without middle name, as the accused;

4. That the above-mentioned accused in both cases was convicted;

5. That the accused in the said cases is while my name is L;

6. That I vehemently deny having any involvement with these cases at the said
Court, as I have no knowledge of such and that I have not committed any crime;

7. That I am not the accused charged and convicted in the above-mentioned


cases;

8. I am executing this affidavit to attest to the truth of the above-cited facts and
for whatever legal purposes this may well serve.
