AFFIDAVIT OF DISCREPANCY

I, Cristina D. Celestino, of legal age, widow, Filipino, and a resident of


Brgy. San Rafael, Tarlac City after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. That my complete name is CRISTINA D. CELESTINO as


entered in our Marriage Contract issued by office of the Civil Registry of
Tarlac City;

2. That recently my son, Rowel Celestino, sent to me a money


remittance in the amount of 21,429.03 under control number
QPAY612181513088;

3. That however, the name indicated by my son in the remittance


form as receiver is CRISTINA CELESTINO instead of my complete name
which is CRISTINA D. CELESTINO;

4. That the name CRISTINA D. CELESTINO and CRISTINA


CELESTINO refer to one and the same person, the herein Affiant;

5. That I am executing this Affidavit to attest to the truth of the


foregoing facts and to use the same for whatever legal purpose it may
serve.
