PETITION
COMES NOW PETITIONER, pro se, respectfully states files for this
Petition for Notarial Commission for and within the jurisdiction of this
Honorable Court, and in amplification thereof, most respectfully aver the
following, to wit:
1. That Petitioner is Filipino, born on January 13, 1991 and a resident of
Poblacion Dictrict II, San Jacinto, Masbate and maintains a regular
place of work in the same address. She may be reached at her mobile
number: (+639567213334). True photocopy of the COMMUNITY TAX
CERTIFICATE issued on May 20, 2022 is herein attached as Annex “A;”
2. That Petitioner has satisfactorily completed the required course for the
Juris Doctor (JD) program (non-thesis) at University of Perpetual Help
System-Dalta. That, petitioner took and passed the 2020-2021
Philippine Bar Exam and was admitted into the practice of law on May
17, 2022 under ROLL NO. 79540. True photocopy of the CERTIFICATE
OF PHILIPPINE BAR MEMBERSHIP issued on May 17, 2022 is herein
attached as Annex “B;”; True photocopy of the OFFICIAL RECEIPT FOR
THE CERTIFICATE OF PHILIPPINE BAR MEMBERSHIP issued on May 17,
2022 is herein attached as Annex “B-1;”
3. That Petitioner is a registered member of the Integrated Bar of the
Philippines shown by a true copy of her CERTIFICATE OF GOOD
STANDING and is herein attached as Annex Annex “C;”
4. That Petitioner has no pending administrative case against her as
evidenced by the Certification dated May 20, 2022 issued by the Office
of the Integrated Bar of the Philippines. True photocopy of the
CERTIFICATE OF NO PENDING ADMINISTRATIVE CASE is herein
attached as Annex “D;”
5. That Petitioner has neither been charged nor convicted before any
court or quasi-judicial body of any crime involving moral turpitude.
Copy of the CLEARANCE FROM THE REGIONAL TRIAL COURT, Branch
50, San Jacinto, Masbate issued on May 23, 2022 is herein attached as
Annex “E;”
6. That Petitioner has not yet applied nor been commissioned as Notary
Public in any other jurisdiction;
7. That Petitioner has paid her Professional Tax Receipt for the year 2022
under Official Receipt No. at Masbate City. True photocopy of
the PROFESSIONAL TAX RECEIPT is herein attached as Annex “F;”
8. That Petitioner prays to be commissioned as Notary Public for and
within the territorial jurisdiction of this Honorable Court, and she
possesses all the qualifications and none of the disqualifications for
said office;
9. That in compliance with the requirements of this Honorable Court for
Notarial Commission, attached hereto are the following annexes and
made an integral part of this petition:
a. Three specimen of her official signature, 3 pcs. Passport sized
pictures with name affixed, as Annex “G;”
b. A plain sheet of paper containing three (3) samples of her
signature and formed an integral part hereof as Annex “H.”
PRAYER
WHEREFORE, foregoing premises considered, Petitioner most respectfully
prays of this Honorable Court that after due notice, publication and hearing, a
Commission as Notary Public for and within the jurisdiction of this Honorable
Court be issued in favor of herein Petitioner.
Concomitant thereto, it is likewise prayed that Petitioner be issued a
Certificate of Authorization to Purchase a Notarial Seal.
Other reliefs just and equitable under the circumstances are likewise
prayed for.

I, ATTY. MARY LIZZA D. BARTOLATA, of legal age, Filipino and a resident
of Poblacion Dictrict II, San Jacinto, Masbate, after having been duly sworn to an
oath in accordance with law, do hereby depose and state that:
1. I am the petitioner in the above-entitled person;
2. I have caused the preparation of the foregoing Petition;
3. The allegations therein are true and correct based on my personal
knowledge and authentic records;
4. To the best of my knowledge, there are no other pending petitions of
the same nature before any court, tribunal or quasi-judicial agency;
and
5. Should I thereafter learn of any similar case pending before any other
court, tribunal or quasi-judicial agency, I shall promptly notify this
court within five (5) days from discovery.
