   Designate Full-time PCO
   Register as Hazardous Waste Generator
   • (Registration Fee of Php 1,040.00 – one time registration)
Responsible for storage and labeling requirements

Submit reports to EMB (SMR and CMR)
   Continue to own and be responsible for the wastes until the
   wastes are adequately treated and disposed
   Prepare Contingency/Emergency Plan
Conduct personnel training
1. If you don’t have a COMPANY REFERENCE ID yet, go to
   http://iis.emb.gov.ph/crs/

 2. Click “User Manual” and follow the instructions to create an
    account, and click “Create an Account”.
Once Steps 1 and 2 are completed, wait for the approval and take
note of the COMPANY REFERENCE ID (EMB ID), DATE
APPROVED and your registers First Name and Last Name
3. Once you already have an approved company reference ID, go to
   https://hwms.emb.gov.ph and click “Click here for Guides (USER
   MANUAL)” for the instructions, and click “Register a new
   account”.

2.     NOTARIZED AFFIDAVIT OF TRUTH AND
       ACCURACY                                A document describing how
                                               the existing hazardous wastes

       CERTIFICATE/APPOINTMENT                 are managed. This may also
                                               include protocols on hazardous

                                               waste handling and storage.
5.     HAZARDOUS WASTE MANAGEMENT PLAN
6.     CONTINGENCY AND EMERGENCY PLAN
The submitted application will be transmitted to the EMB
                   Regional Office
  The EMB case handler will evaluate the application
  If there are lacking or additional documents required
for further evaluation, the application will be returned to
                         the client.
 If all documents are acceptable, client may proceed to
                       the next step.
 An ORDER OF PAYMENT will be issued (Php 1,040.00).
  The Official Receipt will be uploaded and submitted.
  Approval of the Registration (up to 7 working days)
According to DAO 2013-22, HW Generators are given 6 to 12 months of

According to DAO 2013-22, HW Generators are given 6 to 12 months of
                           storage time.
                                              The Hazardous Waste Manifest is
                                              a document issued by the EMB
                                              Regional Office wherein the name
                                              of accredited Transporter and TSD
                                              Facility will be indicated together
                                              with the date of transport, type of
                                              Haz Waste, volume of Haz Waste,
                                              type of vehicle, name of driver.
                                HAZARDOUS     This document should always be
                                  WASTE       in-transit with the transported
                                 MANIFEST
                                              hazardous wastes.

            WASTE GENERATORS
Failure to submit a completed copy of
Hazardous Waste Manifest Form to the
DENR

Performs the functions of a TSD Facility
without the appropriate TSD Facility

Conveys or transports hazardous wastes without the
proper manifest forms

Conveys or transports hazardous in transports not
suitable for the hazardous waste being transported

                          TSD FACILITIES
Accepts hazardous wastes without the proper manifest
(Penalty: Php 50,000.00)
Stores, recycles, reprocesses, treats or disposes of hazardous
wastes at TSD Facility without the appropriate TSD Facility
Permit
(Penalty: Php 50,000.00)
Failure to notify the DENR of the residuals generated as a
consequence of its recycling, reprocessing or treatment
activities
(Penalty: Php 50,000.00)
