SWORN STATEMENT

I, MARISSA A. ALVAREZ, of legal age, married, Filipino and with residential and post
office address at B19 L3 Brgy. Estrella, San Pedro Laguna, after having been duly sworn to in
accordance with law, hereby depose and say;

1. That, I am the mother of TOM A. ALVAREZ born on April 12, 1996 in San Pedro,
Laguna;

2. That, in my son’s Certificate of Live Birth his name registered as TOM A. ALVAREZ
where the middle Initial stands for AYLEON for it was my Last name when I was single
as appearing in the attached Certificate of Live Birth:

3. That, verified by this affidavit that TOM A. ALVAREZ and TOM AYLEON ALVAREZ
refers to the one and the same persons;

4. That I am executing this affidavit in order to attest to the truth of the foregoing
statements, to inform the authorities concerned and for all legal intents and purposes this
may serve best;
