I,MARIA ISABEL L. PABLO,of legal age,Filipino Citizen, and presently residing
at 2 Block 6 lot 67 Tambakol st. Dagat-Dagatan Caloocan City after having been
sworn to in accordance with law do hereby depose and say;

1. That I am graduated on April 3, 2009 at MACARIO ASISITIO SR. HIGH


SCHOOL SY 2008-2009.

2. That I am not enrolled to any University up to present.

3. That I am willing to enroll now to CITY OF MALABON UNIVERSITY to continue


my studying in college.

4,THAT, I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever purpose it may deem serve.

FATHER

I, NELSON DIESTA HANAPIN, of legal age, married Filipino, with residence


and postal address at Blk 32 E PH-3 F1 Kapak St. Dagat-dagatan Caloocan City , under
oath, depose and state:
1. That I am the biological father of HONEY LIN EM ;

2. That the biological mother of my child is IMELDA ADUANA EM;

3. That my child, HONEY LIN EM, was born on April 23, 2001 at Paco, Gubat,
Sorsogon;

4. That the facts of birth of my child HONEY LIN EM, was duly registered at the
Office of the Civil Registrar of Gubat, Sorsogon, under Registry Number 2001-1041;

5. That I, being the father of HONEY LIN EM had acknowledged and recognized her
as my child and legal heir;

6. That I am allowing my child, HONEY LIN EM, to use my surname HANAPIN, to


be her surname, in her Certificate of Live Birth, school records and legal
undertakings;

7. That I am executing this affidavit to attest to the truth of the facts for whatever legal
intents and purposes it may serve and in compliance with the provision of Republic
Act No. 9255.

IN WITNESS WHEREOF, I have hereunto set my hands this 4 th day of July, 2011 in
