AFFIDAVIT
I, ALLAN J. AMBROCE, of legal age, single and a resident of
Brgy. San Blas, Villasis, Pangasinan, after having been duly sworn to
in accordance with law, hereby depose and say, that:

That I have been employed as teacher with “Saint Anthony


Abbot Academy” which is under the Diocesan Schools of Urdaneta
since June 1, 2018 up to present;

That I am designated as coach of ARNIS – GIRLS


(Secondary), who will participate in the 2019 R1AA;

That all the athletes therefore are not members of the


National Team, National Training Pool and Development Pool
receiving monthly stipend/allowance from the Philippine Sports
Commission (PSC);

That all the athletes of ARNIS – GIRLS (Secondary), which are


under me who will participate in the 2019 R1AA are eligible;

That I am executing this affidavit to attest to the truthfulness


of the foregoing statements and for all legal intents and purposes
this may serve;

JOINT- AFFIDAVIT OF DISCREPANCY

We, MYRNA N. MADRID and ESTELITA M. ESLAVA, both of


legal ages, Filipino citizens, married and widow, both residents of
Brgy. San Blas, Villasis, Pangasinan, after having been duly sworn to
an oath in accordance with law, hereby depose and say, that:

We know personally Spouses SONNY S, BAYYA and ELISA O.


DELA CRUZ, they being our close friends and neighbours;

We both know that said Spouses begot a child named SAIMON


DELA CRUZ BAYYA;

We have personal knowledge that said child is using “DELA


CRUZ” as his family name in all his Public and Private documents
and legal transaction with his firm and honest belief that it was his
true and correct family name;

However, when he secured a copy of his Certificate of Live


Birth before National Statistics Office he found out that his
registered family name is indicated therein as “DE LA CRUZ”;

We are executing this affidavit to attest to the truth of the


foregoing and to prove that the person who has a Family name of
“DE LA CRUZ” as per Certificate of Live Birth and the person who
has a family name of “DELA CRUZ” as per Public and Private
documents and legal transactions refer to one and the same person;
