With reference to the Subject cited above I here with informing and instruction to Sri. N.
Narayana Pharmacist, 104 (FDHS) Vehicle instructed to work at Cold Chain Point Area Hospital
Gajwel for Two Days ( Wednesday, and Saturday), and Other four days are attend at PHC
Sirigiripally Siddipet Dist until further orders. the remaining days should attend in regular duties
at PHC Srigiripally.

The Medical Officer PHC Sirigiripally are instructed to relieve the above Individual for
two days (Wednesday, and Saturday), with necessary instructions by making local
arrangements.

With reference to the subject cited above,Sri. Narayana Pharmacist (104 FDHS) phc
Srigiripally, Siddipet Dist deputed to PHC Ahmadipur,Siddipet District Temporarily until further
orders.

The Medical Officer PHC Srigiripally is instructed to relieve the above individual with
necessary instructions by making local arrangements.

And the above individual is instructed to take the Complete Charge of PHC Ahmadipur and
take complete charge from Smt.Ruthu Pramila Pharmacist of PHC Ahmadipur,Siddipet District
immediately and also instructed to Daily Activities in OP and Update daily P Form, E- Aushadi
and Evin Portal , maintain Stock and Issue registers of PHC Ahmadipur.

over the complete charge of PHC Ahmadipur to Sri Narayana , Pharmacist (104 FDHS) of PHC
Srigiripally. And Smt.Ruthu Pramila Pharmacist Of PHC Ahmadipur instructed to look after
complete charge Cold Chain Point (CCP) DH Gajwel , Siddipet Untill Further orders. and
maintain sufficient stock and issue registers and EVIN Portal and any relevant registers.
