DESCRIPTION OF THE PATIENT

Patient is aware that socialization is a termination stage. He still well groomed. Look
very happy to our Socialization. And is very cooperative in every game we held. He is
still calm and can still answer my questions.

DESCRIPTION OF ENVIRONMENT

The weather was calm and sunny. The termination phase was quite more joyous and
active since it is the last time to interact with them. Every patient cooperated and tried
their best to achieve the goals in that particular activity. Mr. E sing. At the end, they are
very happy to have us and we feel the same way. We have enjoyed serving them
because we have learned a lot.

OBJECTIVES

• To evaluate the patient’s readiness of oneself


• To stabilize her good mental health or state of mind
