Objectives:  Be able to establish rapport and trust with the student nurse,  be able to get the patients initial data, profile and background,  Be able to establish the plan of activities and the contract to the patient.

Patient: matagal na.. 3 years na. nung December 8 Nurse: dinadalaw naman po kayo ng mga kapamilya mo? Patient: oo, ung pinsan ko lang. Nurse: talaga ho? Patient: oo, dinalaw nya ako kaso matagal na iyon. Nurse: Alam nyo po ba kung nasaan kayo ngayon? Patient: ako ay nasa pavilion 19, National Center for Mental Health Nurse: ang galing naman Sir. Summary: During Nurse patient interaction, the patients grooming was not good prior to morning care he wear dirty ward gown without slippers. But most of the time he exhibited appropriate facial expression and posture during interactions. He also displays and maintains good eye contact and show interest on the topic.

Evaluation:
The goals in this NPI are partially met because i was able to gather some of the informations about the patient. I was able to understand the feelings of the patient. As a nurse, I let my patient feel that he is not alone and I think its one of the most important roles of a nurse. I think my patient trusts me because he followed whatever I say and he started talking to me as if I not new to him. All I can say is that therapeutic communication is very important in establishing rapport to the patient.
