JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, Dominador B. Credo and Jr. Gerry A. Lingamen both of legal ages, Filipino, and resident of
Phase 2 Progress Homes Subd, Del Rosario, Canaman Camarines Sur. and Zone 4 Sagrada
Minalabac Camarines Sur. respectively, after having been jointly sworn to in accordance with
law do hereby depose and state, that:
1. We personally know the person of Arbheal D. Mancilla who was born on May,30,1996
at Brgy. Cobangbang. Daet Camarines sur. to the spouse Alma Quientla Delbarrio and Ronald
Bellen Mancilla.

2. It was discovered lately that the DATE OF MARRIAGE, AUGUST 4, 1998 was
erroneously entered as JANUARY 9, 1996 ; in his Certificate of Birth.

3. That the true and correct is DATE OF MARRIAGE, AUGUST 4, 1998

4. AUGUST 4, 1998 (Correct Entry) and JANUARY 9, 1996 (Erroneous entry) refer
to one and the same person;

5. That we are executing this affidavit to attest to the foregoing narration of facts and for
whatever legal purposes and intents this may serve.
