Document proving Travel medical ins urance

Travel medical insurance shall cover any expe ns es, which might arise in connection
with repatriation for medica l reasons, urgent medical attention and/or emergency hospital
treatment or death, during their stay in the R epublic of Lithua nia. The insurance shall be
valid throughout the territory of the M ember States and cover the entire period of the
person’s stay. The minimum covera ge shall be EUR 30 000.

The mediation letter (request to issue a multiple-entry national visa to a applicants) of the

applicant comes, submitted in accorda nce with the establis hed procedure
The Letter of M ediation is submitted electronically via the Information S ystem o f
Electronic Services (https://epis.vrm.lt/epis). The electronic information system
provides the mediation letter with the number, which the foreigner indicates upo n
arriving at the V AC.

8 Proof of fina ncial mea ns (e.g., a personal bank account statement) and (or) a regular
income (e.g., a labour contract):
to live in the R epublic of Lithuania (sufficient mea ns of subsistence equal to 1
minimum monthly wage per month);
to return to the country of his or her origin or a foreign c ountry to which the
alien has the right to depart (sufficient a mou nt of funds required to return equals
to 1 minimum monthly wa ge).
9 Proof of accommodation for the whole duration of the intended stay (e.g. document from
the establis hment providing accommodation, proof of private accommodation or a ny other
appropriate document indicating the accommodation envisaged in accordance with the
legislation of the M ember State concerned).
10 SGK registration and s ervice document (SGK tescil ve hizmet dökümü) with readable QR
code.
11 Criminal Record. (with Barcode)
12 Copy of Identity Card or Residence Permit.
13 Copy of passport’s first page and all of stamped or containing visas pages are required.
ATTENTION
Pursuant to Article 23 of Visa Act, visa applications may be concluded in 15 calendar days following the
date Embassy begins evalua ting them. H owever, in some cases, this process may be extended up to 30 days
or 60 days.
In some exceptiona l cases, Embassy may demand additional doc uments in addition to aforementioned ones.
Visa application fee is nonrefundable.
Aforementione d documents m ust be up-t o-date at least one week before application date. The older
doc uments are unacceptable.
Officers of VFS Global reserve their right to ask visa applicants questions abo ut their trave ls during the
application.
I acknowledge that I submit my visa application in 15 days before my planned entry in Sch engen Zone and
th erefore my application may not be con cluded until my travel date.
