with postal address at B44 Lot 7 Hacienda La Joya Buhay na Tubig, Imus City, Cavite do hereby name, constitute and
appoint CAREN C. SUSTITUIDO of legal age, Filipino Citizen, married and with residence address Blk 16 Lot 15
EVERLASTING HOMES TALON CUATRO, LAS PINAS CITY, to be my lawful attorney-in-fact, to act for and in our
behalf and stead to exercise and perform any and all of the foregoing authorities power and responsibilities;
1. To do matters and things whatsoever in or about our property and affairs, or to concur with persons jointly interested with myself
therein doing all acts and deeds herein, either particularly or generally described, as fully and effectually to all intents and
purposes as I could so in my own proper person if personally present.
2. To represent me before the Bureau of Internal Revenue, Registry of Deeds, Assessor’s and Treasurer’s Office of appropriate local
government units, and any all-government offices, in order to process, execute pay relevant taxes and fees and or give effect to the
foregoing authority.
3. To execute any documents in favor of any third person, natural or judicial, upon such terms and conditions that my Atty-in-fact
may deem best; EXCLUDING THE TRANSFER OF TITLE/OWNERSHIP
4. To pay all the necessary fees/taxes/dues and all other fees that may occur in the future and to settle problems that may arise;
5. To perform all other acts necessary to carry out the above purposes; to transact to Assessor’s office for request of certified
true copy of tax declaration, tax clearance and CNI.
HEREBY GIVING AND GRANTING unto my said attorney in fact full power and authority to do and perform
any and every act and thing whatsoever requisite and necessary to be done in about the premises as, fully to all intents and
purpose as I might or could lawfully personally present, with all powers of substitution and revocation, hereby ratifying and
