AFFIDAVIT OF INCOME DECLARATION


I, ALEXANDER C. ALICUMAN, of legal age, Filipino, and a resident of Silangan, San
Mateo, Rizal, Philippines, after having been duly sworn to in accordance with law, do hereby
depose and say:

1. That I am currently earning income as a Construction Worker,

2. As such, on average, I am receiving only Twenty Thousand Pesos (Php. 20,000.00) a


month,

3. That I am executing this Affidavit of Income Declaration as Iskolar ni Mayor


requirement and for all legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 14th day of August
