AFFIDAVIT OF LOSS
I, JOY D. BORJA, of legal age, Filipino, and with residence at Bgy. Poblacion,
Dumaran, Palawan, after being sworn in accordance with law, do hereby depose and
say:
1. That my Brother Jeric D. Borja is the registered owner of a SIM card with the
mobile number 09101302558.
2. That this SIM card has been in my possession and used primarily for receiving
One-Time Passwords (OTPs) for various financial and governmental services,
including but not limited to my accounts with Landbank, UnionBank, GCash, and
other government-related transactions.
3. That on November 1, 2024, my cellphone, along with the SIM card mentioned
above, was lost. Despite thorough efforts to locate it, I was unable to recover the
phone or the SIM card.
4. That I am executing this affidavit to attest to the loss of the said SIM card and to
comply with any necessary requirements for replacement and updating of my
contact information in relevant institutions.
5. That I am aware that this affidavit will be used as supporting documentation for
the purposes of securing a replacement SIM card and updating my records with
affected institutions.
