AFFIDAVIT OF LOSS

I, Marry Jane C. Santos, of legal age, single/married and residing at 662B, Amapola
St., Sta. Rita, Olongapo City after having been duly sworn to according to law hereby depose
and state:

1. That I am the owner of a Mobile Phone model HonorX9A 5G;


2. That on the 22nd of January, 2025 my said Mobile Phone was placed inside my
handbag as I was buying grocery at Olongapo Public Market (New).
3. That as I reached home that the same day, I was shocked as it was not there
anymore;
4. That after diligent search inside my bag, my said Mobile Phone was nowhere to
be found, instead merely my phone case was there.
5. That I am executing this affidavit in order to attest to the truth of the foregoing
circumstances and for the purpose of reporting the loss to the Globe
Telecommunication Company.
