Despite searching, the SIM card could not be found and is believed to be lost beyond recovery.

AFFIDAVIT OF LOSS

I, ___, of legal age, Filipino, married, residing at __, after being sworn in
accordance with law, hereby depose and state:

1. I am the owner of sim card with number __ with Globe Telecom


as my service provider;

2. I placed the sim card, together with other sim cards, between
the phone protector and the back portion of my cellular phone;

3. On __, when I was replacing my sim card of my cellular phone, I


found out that my sim card with the above number was lost.

4. Despite earnest and diligent efforts to locate the same, it could


no longer be found, and I believe that said sim card is now lost
and beyond recovery.

5. I am executing this affidavit to attest to the truth of the


foregoing for the purpose of replacement by Globe Telecom of
said sim card with the same number, and for whatever legal
purpose it may serve.
