PETITION FOR NOTARIAL COMMISSION

COMES NOW, the undersigned Mariel C. Abiera Filipino, of legal


age, a member in good standing of the Philippine Bar, and a resident
of Brgy. Simminublan, San Narciso, Zambales and unto this
Honorable Court, respectfully states:

1. That Petitioner is a duly licensed and practicing lawyer in the


Philippines, admitted to the Bar on November 10, 2024,
with Roll of Attorneys No. 123456, and a member in good
standing of the Integrated Bar of the Philippines, with
current IBP Official Receipt No. 2024-0404 dated
November 10, 2024;

2. That Petitioner maintains a regular place of work or business


at Brgy. Rizal, Olongapo City within Zambales, and intends
to perform notarial acts within said jurisdiction;

3. That Petitioner has not been convicted in the first instance


of any crime involving moral turpitude, and has no pending
administrative or criminal case before any court, tribunal, or
agency;

4. That Petitioner possesses all the qualifications and none of


the disqualifications under Rule III of the 2004 Rules on
Notarial Practice;

5. That Petitioner respectfully applies for a new notarial


commission for the period prescribed by law;

6. That in compliance with the requirements of the Rules, the


following documents are attached hereto as integral parts of
this Petition:

a. IBP Certificate of Good Standing;


b. Clearance or Certificate of No Pending Case from the Office
of the Bar Confidant;
c. NBI Clearance;
d. Photocopy of the Petitioner’s current Professional Tax
Receipt (PTR);
e. Proof of payment of notarial fees;
f. Photocopy of Lawyer’s ID or PRC ID;
g. Two (2) passport-size photographs;
h. Sample signature and seal.

7. That the Petitioner undertakes to faithfully observe and


comply with the duties and obligations of a notary public as
mandated by law and the Rules.

WHEREFORE, premises considered, it is respectfully prayed that the


Honorable Court grant this Petition for Notarial Commission and
issue a Notarial Commission in favor of the Petitioner to act as a
Notary Public for and within Zambales, for the period prescribed by
the 2004 Rules on Notarial Practice.

Other reliefs just and equitable under the premises are likewise
prayed for.
