AFFIDAVIT

resident of Upper Tiparak, Tambulig, Zamboanga del Sur, after having
been sworn to in accordance with law, hereby depose and say:

1. That I am an employee of __________________ as Service driver


sometime in the year 1974 to 1999;

2. That I am executing this affidavit to attest to the truthfulness of


the foregoing facts and to support for my claims benefits from
the Social Security System (SSS) and for whatever legal purpose
this affidavit may serve best.
