Dear Mr. Webster,

RE: Alison Cooper, Year 5 Student

May I refer to you theThis is to refer the above mentioned student for further psychological
assessment., whose father had passed away, 18 months ago due to motor accident. Aside from that, she
has only few friends who often teased her for being overweight and hands gloving with chronic eczema.
She has been showing signs of behavioral changes since her father’s death, 18 months ago.

Alison is the middle child of three siblings and . Hher mother is a full time bank manager. She has a
paternal grandmother, living nearby school who assisted their needs and took care of them especially in
time of sickness.It was reported that Alison has become withdrawn at home and still grieving over her
loss. Apparently, she comforts herself with junk food and now she is overweight.

From her school record, it was stated that Alison was doing well in her first 2 years. But However, since
Grade 3, her level of concentration has been altered and school performance declined. She was limiting
herself from participating in the class activities unless being motivated. In addition, she also refrained
from joining the sports events at school that prompted her school work to decline.

Apart from that, With pertains to her school medical health record at the school shows that she has
center, a repeated visits was being noted (almost every month from the death of her father), with
complaints of recurrent headache and other reasons. Her self esteem is also affected as she is being
teased about her eczema on her hands.Moreover, Alison’s mother had claimed that her daughter is
becoming withdrawn at home and often cried while mentioning his father as she misses him. Then,
diverting such grief by eating chips and cakes after school.
