Near miss

Near Miss – Aluminum scaffold ladder suddenely fellen on ground.

Description (what happened?) Photos / sketches

Location: Amar Building mezzanine floor (Leb room)


Decsription: An aluminum scaffold ladder fellen
from its position due to being left unsecured in a
vertical state. Additionally, neaeby materials became
entangled with the ladder, causing it to topple.

Consequences

(1) Worker might have got injuired (due to fall enjuiry).

Why the accident happened (THE MAIN SYSTEM DEFECTS)?

What did not work: (1) CMO/PMC was not aware of the task.

CAPA System

Corrective Action : 1. Secure the work area remove and inspected fellen ladder, and ensure no future risk.
2. Inspected all ladder and scaffolding, Check for proper stability, locking machanisms and positioning.
3. Conducd a toolbox talk with workers on the importance of securing ladder.
4. Assign a supervisor to oversee sfaffold- related activites for compliance.
Preventive Action : 1. Conduct daily and weekly checks on all scaffolding and ladder placement.
2. provide re-fresher training on ladder safety and fall prevention.
3. Ensure the ground is stable before setting up laddre and scaffold materails.

Key lessons learned / follow-up actions for All Sites (defined by Europe/IMEA/Asia HSE Lead Team)

Key learnings: 1. Regular check of scaffold and ladder can identify potential hazard before use.
2. Always secure ladder using locking mechanisms or tie-off point prevent fall .

Mandatory Actions: 1) Site safety procedure to be explained.


2) Trainig to be provided with all crew member and site supervisor.
3) Area should be clear for multiple activites.
