AFFIDAVIT OF CONSENT FOR ADOPTION

I, RAY ARISTEO C. OSUNA, of legal age, Filipino, married, and


resident of 01-07, Rodano St., Heights, Malaybalay City, Bukidnon, after
being duly sworn to in accordance with law, do hereby depose and state
that:
1. I am the biological father of Ma. Xzkayli S. Osuna, (minor) who was
born out of marriage to Leomelyn Saribio-Nguyen on February 1,
2007 at Malaybalay Provincial Hospital. Attached herewith is a
machine copy of her NSO Birth Certificate whom my signature can
be found acknowledging her as my daughter as Annex “1”;

2. Leomelyn Saribio-Nguyen is already married to Vu Tuan Nguyen, a


Swiss National and lives in Switzerland until now;

3. Mr. Vu Tuan Nguyen wanted to bring her wife and adopt my


daughter and live in his country as one family;

4. Mr. & Mrs. Nguyen asked my consent to allow them to bring my


daughter and that Mr. Vu Tuan Nguyen to adopt my daughter with
an assurance that they will support my daughter on her daily needs
especially her education and treat her as their own child;

5. I am giving my CONSENT freely without threat/duress and without


reservation to let my daughter go with her mother and be adopted
by Mr. Vu Tuan Nguyen to Switzerland and to live there and finish
her education with the support and care of Mr. & Mrs. Nguyen;

6. I am executing this affidavit to attest to the veracity of the foregoing


facts and for all legal intents and purpose this may serve
