Do’s & Don’ts
This module will be used to train the drivers of
our all incoming and outgoing transport
vehicles
These are relevant safety essentials aimed at achieving vehicle-
related zero incident inside our plant premises and truck yard, 100%
compliance of which is mandatory for every driver
Do’s
Park the vehicle in take-off mode only at directed space in
designated parking area. Reverse carefully (if needed), taking
help of traffic marshal/ security person/ another driver
standing outside
While parking, ensure maintaining minimum
distance of 1.5 meter between two adjacent
vehicles
At truck yard, remove ignition key, apply hand
brake before coming out of the cabin of a
parked/ stopped vehicle and then apply wheel
chokes at both sides of wheel
Wear reflective jacket at truck yard before
alighting from vehicle
Go to designated rest shelter following marked
pedestrian walkway at truck yard for taking rest
Make yourself aware about plant safety rules
before entering inside plant premises
Wearing all mandatory PPEs (Safety shoes, helmet, reflective
jacket, safety goggles) inside plant premises is compulsory
Ensure healthiness of vehicle by checking functionality of
brake, tyres, horn, reverse horn, head and tail lights, indicators,
seat belt, mirrors, RUPD, SUPD, reflective tapes on all sides,
wiper, floor hole and communicate to the transporter to rectify
if not found okay
Inspect all sides to ensure absence of anybody in
close vicinity before getting into vehicle to start,
Blow horn twice before starting the vehicle.
Incident-1
What Happened?
On September 9, 2023, at 11:15 AM, a fully loaded Transit Mixer truck arrived from the batching plant
and halted near the QC lab for sampling. An assistant from the QC lab was tasked with collecting the
sample. The QC lab assistant positioned the sample collection tray behind the Transit Mixer truck and
then departed. The Transit Mixer truck driver handed over the sample and returned to the driver's
seat, while the Transit Mixer truck helper sealed the transit mixer and left the location.
Around the same time that the sample collection was in progress, a Wheel Loader, stationed at the
RMC (Ready-Mix Concrete) section, was requested by the Building Product Division (BPD) plant for a
material movement task. To access the standard route for the movement toward the BPD plant, the
Wheel Loader operator relocated a barricade that had originally separated the raw material bin from
the pathway. Subsequently, the Wheel Loader was driven forward and came to a stop behind the
Transit Mixer truck. Meanwhile, the QC lab assistant, who had initially left the sample collection tray,
returned to retrieve it.
As the Transit Mixer truck driver began moving forward in preparation to leave the area, the Wheel
Loader operator also initiated movement without being aware of the QC lab assistant’s presence at
the location. Tragically, the Wheel Loader ran over the assistant. Promptly, the injured assistant was
transferred to the hospital for the necessary medical attention. Regrettably, despite all efforts, his
injuries proved fatal, and he succumbed on September 10, 2023.
Recommended corrective actions:
Make yourself aware about plant safety rules before entering inside plant
premises.
Make everybody alert by blowing horn twice before starting any stopped/
parked vehicle inside plant.
Always keep vigil watch on road to avoid collision with anybody.
Don’t use mobile phone in any form (making/ responding to call, sending/
receiving message, listening to audio, watching video) while driving inside
plant.
Wearing headphone/ earphone while driving inside plant is prohibited.
After getting out of truck yard, stop the vehicle at main
entry gate of plant and help to duly get checked by Security
person.
Use functional seat-belt all the time while
driving and always keep vigil watch on road to
avoid collision with anybody/ another vehicle
While driving vehicle inside plant premises,
follow all road & driving safety rules as per
visuals and signages displayed
Drive slowly and cautiously at turning point/ bend
Reverse vehicle carefully at loading area as
signalled/directed by Security person/ traffic marshal
Inside plant premises, remove ignition key, apply hand brake
before coming out of the cabin of a parked/ stopped vehicle
and ensure applying wheel chokes at both sides of wheel
Stay inside the cabin of the vehicle at
loading area, give the key to the operator
Follow the instructed key policy at unloading area, give
the ignition key to the operator
Sit at the designated place only during
unloading of material from vehicle
Stay inside the cabin of the vehicle during waiting /
loading inside plant. Only go to either canteen or rest
room if needed following designated pedestrian
walkway
Roaming around inside plant after getting out of vehicle and
approaching a moving/ rolling vehicle is strictly prohibited.
Incident- 2
What Happened?
A 14-wheeler truck was being loaded with cement bags at loading point no. 1.
After completing the 12th row, the tally checker instructed the driver to move
forward for the 13th and final row. The driver took the key from LOTO box and
started the engine. However, the truck didn't move. Meanwhile, the loaders
completed the stacking and left to take a break. A few minutes later, the driver
got off the truck without stopping the engine, applying the hand brake, or using
the wheel chock. The truck started rolling forward on a slight incline towards the
right side. The driver noticed and tried to stop the truck by running alongside it
from the left side and moved to the right side (driving side) by crossing from front
side of the truck, to access the cabin for arresting vehicle’s movement. Tragically,
he got caught between the side handrail of the shed and the rolling truck before
he could get into the cabin, resulting in severe injuries. Despite immediate first
aid, he later passed away at the hospital in Tilda.
Recommended corrective actions:
• Make yourself aware about plant safety rules before entering inside plant
premises.
• Stay inside the cabin of the vehicle during waiting / loading inside plant. Don’t
get down from the cabin of the vehicle.
• If unavoidable, remove ignition key, apply hand brake before coming out of the
cabin of a parked/ stopped vehicle and ensure applying wheel chokes at both
sides of wheel.
• Never approach a moving/ rolling vehicle, always keep safe distance from it.
Climbing on top of vehicle is strictly prohibited. Use the
given facility of platform and lifeline for tarpaulin tying/
removal activity wearing safety harness
Don’ts
Parking vehicle on slope is prohibited
Don’t park vehicle beneath overhead electrical line
Don’t sit / lie down/ rest below truck or on the ground
in either truck yard or plant.
Except authorized driver, entry of any helper/
cleaner in vehicle inside plant is not allowed
Bringing highly inflammable substances such as gas
cylinder, stove etc. and/ or explosive substances
inside plant is prohibited
Don’t bring alcohol/ drug inside truck yard/ plant premises.
Driving and/ or entry inside plant/ truck yard is not allowed
if under influence of alcohol/ drug
Using mobile phone in any form (making/ responding to
call, sending/ receiving message, listening to audio,
watching video) and wearing headphone/ earphone while
driving with is prohibited
Over-speeding of vehicle beyond displayed speed
limit inside plant premises is punishable offence
Km/ hour
Overtaking another vehicle inside plant is prohibited
Don’t climb on top of any vehicle without using
fall prevention/ protection device
Whatever be the situation, never go below truck
दुर्घटना- 3
What happened ?
The driver (IP) reported at Gate No. 7 around 15:05 hours on 13.12.2023 with
laterite material truck, parked the truck on the approach road towards the old
weighbridge and started waiting for his turn.
At 15:18 Hours, auto-plant technician called driver for weighment at new
weighbridge as cleaning was under progress at old weighbridge.
IP reversed his truck and positioned towards new weighbridge. IP’s turn came for
weighment, however instead of moving his truck to weighbridge, he got down from
his truck without switching-off engine, engaging hand brake and applying wheel
chokes. IP bent to locate his mobile from driver side and then moved towards non-
driver side.
He entered under the chassis (just behind front tyre) to retrieve his mobile. After 50
seconds the truck rolled back. IP got stuck by wheel on left side of his lower
abdomen.
IP was immediately shifted to Occupational Health Centre and subsequently to
Anantapur for further treatment. He succumbed to his injuries on 16.12.2023 @
00:15 hours.

Recommended corrective actions:
• Ensure that there is no hole/ opening in the floor of vehicle cabin.
• Don’t use mobile phone in any form (making/ responding to call, sending/
receiving message, listening to audio, watching video) while driving inside
plant.
• Don’t park vehicle anywhere else inside plant premises other than designated
place.
• Stay inside the cabin of the vehicle during waiting / loading inside plant.
Don’t get down from the cabin of the vehicle. If unavoidable, remove
ignition key, apply hand brake before coming out of the cabin of a parked/
stopped vehicle and ensure applying wheel chokes at both sides of wheel.
• Whatever be the situation, never go below truck.
Cooking inside cabin of or under the truck /
nearby the truck is prohibited
In case of breakdown of vehicle, don’t try to engage in
repair/ maintenance of it. Inform responsible officer of
UltraTech for maintenance by competent mechanic
